TARGET AUDIENCE

Family Physicians, Primary Care Nurses, Social Workers, Pharmacists, Students, Residents, Geriatricians and Other Health Professionals.

LEARNING OBJECTIVES 

By attending this activity, participants will be able to: 

  • Describe the unique medical health care needs of different elderly populations (e.g. Indigenous, LGBTQ, Homeless and shelter, and Long-term care) 
  • Describe the unique social perspectives impacting health for different elderly populations (e.g. Indigenous, LGBTQ, Homeless and shelter, and Long-term care) 
  • Describe the role of a new tool that can identify opportunities for advance care planning conversations in those with serious illness 
  • Identify the indicators used to assess quality of care in Long-Term Care homes in Ontario
